imPACT Concussion Testing

Attention current 8th graders! It is recommended that athletes re-establish their Baseline ImPACT test as they enter high school. Any athlete who does not have a baseline, can also participate in the testing. The ImPACT Concussion baseline tests will be administered during Middle School PE classes during the last cycle of the school year. If you would like your son or daughter to participate in this testing, they must bring a completed permission form to the computer lab on the date of their testing. The form says to turn the permission slip into the UHS Athletic Office, please disregard that and have your child bring the form to PE on the day we are testing them. They will not be permitted to take the test without the signed permission slip in hand. The form is available by clicking here and copies will also be in the main office at CFPMS.
