Boys Basketball Tryouts

Attention Parents of Boy's Basketball players. If your son is interested in trying out for basketball, all medical forms are due to the Main Office by December 2nd. Between 7th and 8th grade, there are more than 100 boys trying out. This deadline will give us time to review the forms, and identify any problems, in advance rather than the first day of tryouts. Nothing is worse than having to tell a young man he can't try out yet because his forms aren't signed or dated or otherwise incomplete or missing. If you are not certain which forms you need, please visit the Athletic Training website. If you have any questions, contact Mrs. Walsh-Shell at kwalsh-shell@ucfsd.net

Any athlete who does not have a baseline, can also participate in the testing. The ImPACT Concussion baseline tests will be administered during Middle School PE classes during the last cycle of the school year. If you would like your son or daughter to participate in this testing, they must bring a completed permission form to the computer lab on the date of their testing. The form says to turn the permission slip into the UHS Athletic Office, please disregard that and have your child bring the form to PE on the day we are testing them. They will not be permitted to take the test without the signed permission slip in hand. The form is available by clicking here and copies will also be in the main office at CFPMS.
